* Extrude without profiles keeps nested loops as holes (#5172)

With no `profiles`, solid.extrude took every region of the sketch, so
circles drawn inside a plate outline were extruded as well and filled the
plate's holes. The default now applies even-odd nesting to the drawn
profiles: a loop inside an outline is its hole, a loop inside that hole is
material again, and separate outlines all extrude. Sketches without nesting
keep the `all` selection, and explicit `profiles` (including "all") are
unchanged. Saved designs store their selection, so they recompute as before.
